These pages provide the necessary forms you will need as well as detailed lists of affiliated hospital sites for both core and elective rotations. Keep in mind that the hospital lists serve as a guide and are subject to change. Therefore, be sure to keep in contact with the Office of Clinical Student Affairs to receive the most up-to-date information on the hospital sites you are interesting in pursuing.
